Deposit Policy : Our goal is to provide quality health care to all our patients in a timely manner. No-shows, late arrivals, and cancellations inconvenience not only our providers, but our other patients as well. In order to limit the number of missed appointments, a non refundable deposit of $25.00 is required at time of scheduling. The deposit of $25.00 will be applied as a credit to your account and will be applied to your final invoice the day of your appointment. If for any reason you need to cancel or reschedule the appointment, you will be required to do so at least 48 hours prior to your appointment in order for the deposit to remain as a credit on your account. Cancellations made without a 48 hour advance notice as well as no show's will result in loss of the deposit.
